LCHD-Hospice Team & Services

LCHD Registered Nurses & Licensed Practical Nurses provide skilled nursing care in the home as approved by the patient’s primary doctor and the Hospice Team.

LCHD Home Care Aides assist with personal care and housekeeping.

LCHD Social Worker helps the patient and family cope with the emotional stresses of an illness.

Chaplain provides pastoral care as requested and assists with coordination of spiritual counseling.

Medical Director provides pain and symptom management.

Volunteers provide companionship and support by assisting with personal needs, reading, errands

Bereavement Support is provided by the LCHD-Hospice Team and volunteers to help family and significant others cope with grieving before, during, and after.

Massage Therapy is available to patient and caregiver to assist with relaxation, stress relief, and pain control.

Nursing Facilities and Assisted Living Homes care is provided in Intermediate Care and Assisted Living Facilities in Hospice’s service area. The facility is considered the patient’s home.

Physical, speech, and occupational therapy, and dietary counseling are also available. All services are provided with the approval of your physician.
