Lee County Board of Health
The Lee County Board of Health will hold a meeting Thursday, November 7, 2024 at 3:30pm at the Lee County Health Department. Please contact the agnecy administrator if unable to attend in person, and call in information can be provided. Click here for the meeting agenda.
Lee County Immunization Records
Under the State of Iowa Administration Code (653-13.7(8)(2008) and 614.8(2008)) immunization records for minor child clients must be retained until 1 year after the child turns 19. If you have received an immunization from LCHD before the year 2000, and would like your record, please click here to fill out the form below. If we are able to locate your record we will contact you. All records will need to be picked up at LCHD (#3 John Bennett Drive) in Fort Madison. No records will be mailed.
Due to the large amount of records, residents will have until December 31, 2024 to put in a record request.
